Coordinating communication between care practitioners while ensuring your loved one is represented competently. Advocacy through the spectrum of Hospital stays, Extended Care and Assisted Living. Supported patient care can decrease the length of hospital stays and improve patient outcomes. Unexpected admission to Hospital is especially disorienting for a person with dementia.
Having a “care-partner” or Advocate to help as an extra pair of eyes and ears when family cannot be present can be very reassuring. In my experience, hospital and nursing staff often express how relieved they are to see their patient getting extra support with private care as they simply do not have the resources to manage the extra needs of the patient with responsive behaviours.
Kelly can also provide consultation, assessment and contribution to care plans in conjunction with health and wellness teams.
